WILDLIFE WATER SCHOOL EMPLOYEE ACKNOWLEDGEMENT
This Employment Acknowledgement is made and entered into as of [Date] by and between Wildlife Water School, LLC("Employer") and[Employee Name]("Employee").
1. Employment Status: The Employee acknowledges that they are hired as apart-time employee. Hours are not guaranteed and will vary based on business needs. This employment is"at-will,"meaning that either the Employee or the Employer may terminate the employment relationship at any time, with or without cause or notice, in compliance with Pennsylvania employment laws.
2. Legal Eligibility & Work Permits:
• Work Authorization:The Employee confirms they are legally eligible to work in the United States and will provide a completed Form I-9.
• Minors (Under 18):If the Employee is under 18 years of age, they must provide a valid Pennsylvania Transferable Work Permit prior to their first shift. Failure to provide this permit will result in the inability to work.
3. Duties and Responsibilities: The Employee agrees to fulfill duties as outlined in the training materials, including adhering to policies regarding safety, communication, and lesson structure.
• The Employee agrees to seek assistance from management (e.g., Victoria Heffelfinger) for questions regarding their role.
• Communication protocols must be respected at all times.
4. Compensation and Tax:
• Training Rate:The Employee will be compensated at a rate of $10.00 per hour for training and instructional time.
• Regular Rate:Hourly rates for teaching, lifeguarding, and other tasks will be set separately and communicated to the Employee.
• Taxes:The Employer will withhold all applicable taxes. The Employee must submit a W-4 and other required tax forms. The Employer will provide a W-2 annually.
5. Workers' Compensation: The Employee is covered by Workers' Compensation Insurance through "Employers Preferred Insurance Company" for work-related injuries. In the event of an injury:
1. Seek Medical Attention:Immediately, if necessary.
2. Report the Injury:You must report the injury to Victoria Heffelfinger immediately(on the same day).
3. Procedure:The Employee must complete the necessary claim forms and follow the treatment plan provided by medical professionals.
6. Compliance with Laws & Breaks: The Employee agrees to comply with all applicable laws, including:
• Non-Discrimination:We uphold a policy of non-discrimination based on race, color, gender, religion, national origin, age, or any other protected status.
• Child Labor Act (Breaks):In accordance with PA law, any employee under 18 working a shift of 5 or more consecutive hours is required to take a documented 30-minute break.
7. Expectations & Conduct:
• Professionalism:Employee must maintain respect towards staff and participants.
• Punctuality:Employee must arrive on time and be prepared.
• Safety:Employee must strictly adhere to safety rules and Emergency Action Plans (EAP).
• Notification:Employee is responsible for informing management immediately of any intention to resign or any injury sustained on duty.
8. Mandatory Clearances: Prior to employment, the Employee must submit the following clearances required for roles involving direct contact with children:
1. PA State Police Criminal Record Check (Act 34)
2. PA Child Abuse History Clearance (Act 151)
3. FBI Criminal History Background Check (Act 114)
9. Electronic Signatures: The parties agree that this acknowledgment may be electronically signed. The parties agree that the electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
